What can I expect when I come to your office?

We strive to make your visit to our office as relaxed and comfortable as possible. You will be greeted by one of our friendly staff in a warm atmosphere with candles and soft music playing. On your first visit we will ask that you arrive 15 minutes early to complete some information for us about you, your family, and your situation. You will then be welcomed by your counselor and taken into their office where you can speak openly and freely.

What paperwork do I need to fill out?

The paperwork that we have our new clients fill out tells us a little about you, things that you are struggling with, and any health issues you have. We also have you complete paperwork that confirms that you know our policies and understand our confidentiality requirements. If you are a new client and wish to complete the paperwork before you come to our office you may download them here using Adobe Reader. You may then either fax the information to us or bring it with you to your appointment.

• Client Application (complete for the client, if a minor for the child)
• Consent for Release or Exchange of Information (this allows us to provide your doctors or insurance company information pertinent to your care)
• Consent to Treat (to be completed only if the client is a minor)
• Information and Professional Services Agreement (this document contains information about our center that we need to be sure you are aware of)

Monday through Friday, 9:00 a.m. to 7:00 p.m. and Saturday appointments available from 9:00 a.m. to 4:00 p.m.
